By completing this form you agree to the personal information you have provided being digitally stored and searchable on the ACTNoW® database (the Database). The Database is hosted by TrusTECH, part of Central Manchester and Manchester Children's University Hospitals NHS Trust (the Trust). The Trust is registered with the Information Commissioner's Office to process personal data of this type.
The Database is accessible via the Internet and can be fully accessed (e.g. by name, specialty or organisation) once individuals have obtained a username and password from us. Your personal contact details will not be available via the database.
Access is granted to healthcare and industry professionals who have valid reason to require data for clinical research site selection purposes. Individuals requesting access to the database will typically be from (but not limited to) the following healthcare related organisations: NHS Trusts; Strategic and/or Special Health Authorities; the Department of Health; pharmaceutical or medical device companies; Clinical Research Organisations.
We may also disclose the information that you have provided in response to lawful requests from regulatory authorities. We will inform you about such disclosures where possible.
You may update or remove your personal information from the Database at any time by informing us in writing, by e-mail, fax or letter.
