Instructions for completing and submitting the ACTNoW stakeholder feedback survey
We are currently in the process of asking stakeholders in ACTNoW for their views and suggestions on how the service should be developed to maximise the potential benefit of using ACTNoW. This online survey, which should only take 5-10 minutes to complete, will ask for your views on the desirability of some suggested new features. You will then be asked further questions depending on the stakeholder group to which you belong relating to how you use ACTNoW currently. Your answers will be crucial in enabling us to target the most important and useful areas of development so I would like to thank you for your time and commitment to ACTNoW.
As a token of our appreciation every 10th survey returned completed will win a 1GB flash memory drive.
The online survey is an adobe form and there are 2 options for filling it in and returning it to me depending on the version of Adobe Acrobat you have installed.
If you are using a computer with Adobe Acrobat Professional installed:
Open the form and fill in the required information. The completed form can be submitted directly by clicking the 'submit by email' button which is found in the top right hand corner of the first page (see screenshot below). Alternatively, you should be able to save the completed form and send it by email, as an attachment, to matthewhobbs@actnow-database.co.uk.
If you are using a computer with Adobe Acrobat Standard or Reader installed:
Open the form and fill in the required information. The completed form should be submitted directly by clicking in the 'submit by email' button which is located in the top right hand corner of the first page (see screenshot below).
Adobe Acrobat Reader can be downloaded for free from Adobe's website.
